Maybe some mothers find that losing weight after childbirth is more difficult than before pregnancy. If you want to reduce the incidence of postpartum obesity or make postpartum weight loss less difficult, "prevention" is the focus. Postpartum obesity has a great relationship with obesity during pregnancy. Therefore, we should pay attention to preventing postpartum obesity from the beginning of pregnancy.

Research data shows that excessive weight gain during pregnancy may cause postpartum weight retention, and even long-term development of postpartum obesity, and eventually cause weight-related diseases [2, 3].

A domestic study on "the effect of excessive weight gain during pregnancy on the weight of postpartum pregnant women" found that excessive weight gain during pregnancy led to a significant increase in weight retention at 42 days and 3 months postpartum [4]. Foreign studies have also found that the level of weight gain during pregnancy is significantly positively correlated with the weight of 42 days postpartum, suggesting that excessive weight gain during pregnancy affects the recovery of postpartum weight, which causes weight retention in women and may affect women’s future health [5]. During pregnancy, how to prevent excessive weight gain during pregnancy?

1. Body weight monitoring

Body weight should be monitored and managed from before pregnancy. The weight in the first trimester does not change much, and it can be measured once a month. The weight should be measured every week in the middle and late pregnancy, and the energy intake level should be adjusted according to the rate of weight growth. Expectant mothers who are thin before pregnancy should gain more weight during pregnancy, while those who are overweight or obese before pregnancy should lose more weight during pregnancy. Please refer to the following table for the suitable range of weight gain during pregnancy for singleton pregnancy:

2. Supplement folic acid and vitamin D

Excessive weight gain during pregnancy is a risk factor for pregnancy complications such as hypertension in pregnancy, gestational diabetes , etc.Thereby increasing the fetal malformation rate, premature birth, giant baby , fetal growth restriction incidence rate and so on. Therefore, pregnant women should take at least 0.4mg of folic acid a day, and multivitamins may reduce the risk of fetal congenital abnormalities and preeclampsia . It is recommended that women supplement folic acid and 400U vitamin D daily from the first 3 months to the first trimester [6, 7]. In addition, you can eat more fresh vegetables and whole grains, especially green leafy vegetables, in your diet. Vegetables contain a lot of vitamins and folic acid; and whole grains are also rich in plant protein and vitamins.

3. Supplement ω-3 fatty acids

A study published in "Am J Clin Nutr" based on the cohort study of Asian women, and derived the plasma ω-3 fatty acids of pregnant women The conclusion is negatively correlated with postpartum weight retention. That is to say, the more ω-3 taken during pregnancy, the less weight will be retained after delivery. The researchers included pregnant women (653 people) in a prospective cohort of from June 2009 to September 2010. At 26 to 28 weeks of pregnancy, the plasma phosphatidylcholine polyunsaturated fatty acid concentration was measured and its proportion to the total fatty acid was determined. The results showed that the median retained weight at 18 months postpartum was 0.90 kg. Among 653 women, 544 (83.3%) retained weight less than 5kg at 18 months postpartum, and 109 (16.7%) retained weight ≥5kg at 18 months postpartum. In the adjusted linear regression model, higher plasma EPA, DHA , and total ω-3 concentrations are correlated with lower body weight retention of at 18 months postpartum, while higher plasma ω-6/ω-3 is associated with postpartum 18 A higher monthly weight retention is associated. Therefore, pregnant women can increase EPA and DHA status through diet or fish seed oil supplementation during pregnancy.Reducing the ratio of ω-6/ω-3 in plasma can provide an alternative strategy for assisting postpartum weight loss.

In the third trimester of pregnancy, 30 minutes of moderate-intensity physical activity should be performed every day. The evaluation of moderate-intensity exercise is as follows: any exercise that causes a little sweating or moderately increases breathing or heart rate; simply calculates the amount of exercise for brisk walking (4-6km /h); exercise intensity during pregnancy reaches 60%-70% of the maximum heart rate (maximum heart rate=220-age). Common moderate-intensity exercises include: brisk walking, swimming, cycling, dancing, pregnant women yoga , various housework, etc., which can improve the insulin sensitivity of , reduce weight, and improve blood sugar and blood lipid levels. Pregnant women can choose the type of activity based on their physical condition and exercise habits before pregnancy, combined with subjective feelings.
