[Anterior interosseous nerve entrapment sign]

is a syndrome caused by the anterior interosseous nerve branch of the median nerve being compressed by the tendon arch or fibrous band at the upper edge of the flexor digital superficialis muscle.

[Clinical manifestations]

1. Pain in the antecubital area, weakened flexion of the distal interphalangeal joints of the thumb and index finger, normal hand feeling, and no paralysis of the inner muscles of the hand.

2, anterior interosseous nerve entrapment sign is a pure motor nerve paralysis, which is characterized by weakening of the flexor pollicis longus , the flexor digitorum profundus of the index finger and middle finger, and the pronator quadratus muscle.

3. The anterior interosseous nerve has a terminal sensory branch that controls part of the wrist. Pain in the forearm and wrist is a common clinical manifestation of this disease.

4. It is more likely to occur in people with forearm and elbow fractures, handicraft workers and keyboard operators, those who have been engaged in heavy physical labor for a long time, or athletes.

【Cause】

1, nerve compression.

The first is abnormal fiber band compression caused by tendon band compression of the pronator teres, flexor digitorum superficialis, flexor carpi radialis and other accessory muscles such as the flexor pollicis longus;

The second one is straddling the Abnormal vascular compression of the accessory blood vessels above the median nerve and anterior interosseous nerve;

The third is abnormal neobiotic compression caused by the enlarged biceps bursa.

2, viral neuritis.

Neuritis invades the median nerve or anterior interosseous nerve, causing nerve cell swelling and entrapment symptoms.

3, nerve entrapment.

Inflammation and injury are the main causes of anterior interosseous nerve entrapment syndrome. Direct injury to the forearm causes injury or compression of the anterior interosseous nerve. For example, when the forearm is fractured, the nerve is directly stretched, or due to fracture hematoma, callus callus formation, leading to nerve entrapment.

[Signs]

1 Spontaneous pain on the volar side of the proximal forearm, pronator teres area and volar side of the wrist. Symptoms are aggravated during movement, especially when the forearm is moving. The symptoms are more obvious.

2. Due to pain, the movement of limbs is restricted. 3. The pain can relieve itself within a few weeks or months.

4, the flexor pollicis longus, the flexor digitorum profundus of the index finger and middle finger, and the pronator quadratus muscle are weakened, making it difficult to write or hold small objects, but there is no change in hand feeling.

【How to deal with it? 】

There are targeted treatments according to different causes.

1. For injuries to the anterior interosseous nerve caused by trauma, the patient should generally be observed for 3 to 4 months. If recovery cannot occur, surgical treatment should be performed.

2. Penetrating nerve injury should be treated immediately with surgery.

3. Non-surgical treatment can include rest, immobilization, reduction of forearm activity and partial closure.

4. Under the guidance of musculoskeletal ultrasound, hyperplastic adhesions compressing nerves can be precisely released, with immediate results.
