"hunchback"? Beware of ankylosing spondylitis!

Medical Guidance/Orthopedics II Orthopedics (Spine Orthopedics) Director Yao Zhensong, Traditional Chinese Medicine Physician

Pictures/Orthopedics II Orthopedics (Spine Orthopedics) Dr. Ren Hui

"I finally straightened my spine and stood up again. Thank you for your medical team's careful care!" Uncle Li, who was hospitalized for half a month and was about to be discharged from the hospital, said excitedly.

I remember that on a busy afternoon more than 10 days ago, a middle-aged uncle walked towards the second orthopedic ward step by step with his back hunched over. Although Uncle Li is only 44 years old, he has been entangled with ankylosing spondylitis for more than 20 years. The spine bone on the waist and back has severe kyphosis, forming a "hunchback" deformity. He has been taking oral painkillers for a long time, but the pain still cannot be relieved. Uncle Li can only curl up and sleep every day. When he moves a little, his waist and back will be in unbearable pain, which has caused great trouble to his life.

Speaking of ankylosing spondylitis, it is actually not far from us. It can be seen everywhere in life. For example, a certain star once suffered from this disease. His hunchback and shaking walking posture once attracted people to imitate it.

So what is ankylosing spondylitis? What are the symptoms of ankylosing spondylitis? How should we treat and prevent?

Ankylosing spondylitis (AS) is a chronic disease that involves the central axis of the human body and belongs to the category of rheumatism. The prevalence rate is about 0.1-1.4%, and the incidence age is 15-40 years old. Among them, there are significantly more male patients than female patients, and the proportion of male and female patients is about 2:1. AS clinically manifests itself as chronic inflammatory changes in large joints such as sacroiliac joints and hip joints. The pathological changes are lesions at the ligament attachment point, resulting in ligament ossification, joint fiber or ankylosing bone, vertebral osteoporosis, muscle stiffness and kyphosis deformity. It is mainly manifested as sagittal imbalance of the spine, resulting in changes in biomechanics , the back muscles are in a stretched state for a long time, the patients are unable to lie flat and walk difficult, and some patients even have respiratory and digestive functions involved. As the condition progresses, AS patients have severe kyphosis in their spine, forming a typical "hunchback", which has great troubles to life and work. They were once called "undead cancer".

Why should you beware of AS when your body has symptoms?

1. Chronic low back pain: is usually more than 3 months, and the early symptoms are mild. As the condition progresses, morning stiffness, fatigue, difficulty turning over, and even pain in the middle of the night, which will relieve after activity.

2. Hip Pain: Patients with teenagers who have chronic hip pain often develop chronic hip pain, which later develops into hip deformities.

3. Heel pain: manifests as redness, swelling and pain in the heel.

4. Swelling and painful eyes: Unexplained eyes are red, painful, blurred vision, and loss of vision. What are the main manifestations of imaging in

AS?

osteophytes fusion, forming a "bamboo joint-like" change (Figure 5 and Figure 6) Can

still be treated if it has AS?

Modern medical technology is becoming more and more advanced, ankylosing spondylitis is no longer terrible. Please seek medical treatment in time if your body is discomfort! The early treatment goal of ankylosing spondylitis is to treat conservatively in combination with traditional Chinese and Western medicine. It is often controlled by taking drugs by taking low back pain, , joint pain, , morning stiffness, fatigue and other symptoms, to slow down the progress of the disease, and to undergo surgical treatment if necessary. If treated irregularly, it is easy to progress gradually, and later "hunchback" deformity occurs due to back pain. For such patients, surgery is often the most reliable and effective way of treatment.

Uncle Li is a patient with late AS. After being admitted to the hospital, we will conduct relevant examinations.Among them, X-rays show that the sacroiliac joint space is narrow, the joint surface is blurred, and the edges are irregular; the vertebral joint is blurred, the spine is changed like bamboo joints and bone bridges; kyphosis of the spine at the waist and back. We confirmed that the disease that Uncle Li was suffering from was ankylosing spondylitis and kyphosis. After detailed assessment of the condition and surgical risks, we performed lumbar kyphosis orthopedics and sufficient decompression spinal canal surgery for him. Just 15 days after the operation, Uncle Li "straightened his back" with the help of the walker and walked out of the ward with a smile...

In short, ankylosing spondylitis is still a chronic nonspecific inflammation of the spine and joints of unknown causes. It is mainly common in middle-aged men, and early manifestations are chronic occult low back pain and spinal stiffness. As the condition progresses, severe kyphosis of the spine occurs. But with the current medical conditions, ankylosing spondylitis is no longer a "cancer". Therefore, it is not terrible to have ankylosing spondylitis. Active symptomatic treatment in the early stage, prevent fractures and kyphosis in the later stage, and perform surgical treatment when necessary, which plays a crucial role in slowing the progression of ankylosing spondylitis.
