4. CPC Information required for the certificate:

2. All regulations and clauses applicable to the product;
3. Manufacturer information: including name, address and telephone number;
4. Product production date and address, production date must reach the year and month, address must reach the city;
5. Testing time and address
6. Third-party testing agency information (a laboratory approved by CPSC): name, address, contact number;
5. CPC required for normal export of children's products to the United States Information required for the certificate:

3. Information of the importer or manufacturer (local United States): including name, address and telephone number;
4. Contact information of the file holder of the test result supporting the certificate: name, address, email address and telephone number;
