Recently, the top international respiratory journal European Respiratory Journal published the latest research results of the Huazhong University of Science and Technology Tian Yaohua's research group of the School of Public Health, Tongji Medical College, entitled "air pollutants, genetic susceptibility and risk of incident idiopathic pulmonary fibrosis" (Air pollutants, genetic susceptibility and risk of incident idiopathic pulmonary fibrosis 4).
Idiopathic pulmonary fibrosis (IPF) is the most common idiopathic interstitial pneumonia , and is a chronic, irreversible progressive fibrotic lung disease. This disease lacks effective therapeutic drugs, and the average survival period of patients with IPF after diagnosis is only 2-4 years. In recent years, more and more research evidence suggests that air pollution may be a risk factor for IPF. Experimental studies have reported that air pollution is related to various pathophysiological mechanisms of IPF. In addition, some epidemiological studies have found that air pollution exposure is associated with increased IPF, hospitalization and death. However, no research has been conducted to explore the association between individual air pollution exposure and IPF onset.
This study included 433,738 study subjects from UK Biobank, and 1,380 cases were confirmed after an average follow-up of 11.8 years. After controlling for a variety of confounding factors, found that for every interquartile spacing of NO2, NOX and PM2.5 exposure concentrations increased by 11% (HR: 1.11, 95% CI: 1.03-1.09), 7% (HR: 1.07, 95% CI: 1.01-1.12) and 9% (HR: 1.09, 95% CI: 1.02-1.17).
Dose-response relationship of the risk of idiopathic pulmonary fibrosis
This study further explores the effect modification of genetic susceptibility on atmospheric pollution-IPF. To study risks, the genetic risk score of IPF constructed based on 13 SNP sites is closely related to IPF incidence and has an additive interaction with NO2, NOX and PM2.5. Compared with the low-gene risk and low-air pollution exposure group, the high-gene risk and high-air pollution exposure group had the highest risk of disease [NO2: 3.94 (2.77, 5.60); NOx: 3.08 (2.21, 4.27); PM2.5: 3.65 (2.60, 5.13); PM10: 3.23 (2.32, 4.50)].

In summary, this study found that for the first time that long-term exposure to air pollution in individuals increased the risk of IPF. There is an additive interaction between genetic factors and air pollution exposure on the risk of IPF.
