It has the advantages of speed, simplicity of use, wide application range, and overall cost savings. .

Fuwai HospitalZhang Jian and others found that the detection of soluble growth-stimulating gene 2 protein (sST2) by POCT method is an independent predictor of all-cause death or heart transplant in patients with heart failure. Based on baseline NT-proBNP, The combined baseline sST2 may provide additional predictive value.

sST2 is a marker of cardiac hypertrophy , fibrosis and myocardial remodeling. It has high application value in the auxiliary diagnosis, prognosis assessment and guidance of the treatment of heart failure. The

study included 747 patients with heart failure. At a median follow-up of 949 days, 231 (30.9%) patients had experienced all-cause death or heart transplantation. Baseline sST2 levels in the overall population were 22.14 (15.04, 34.67) ng/ml.

Patients were divided into case group (n=231) and control group (n=516) according to whether the main clinical endpoint event occurred during hospitalization or follow-up period. The primary clinical endpoint was death from any cause or heart transplantation.

Baseline sST2 levels were higher in the case group compared with the control group.

Multivariate Cox regression analysis results showed that baseline sST2 was an independent predictor of all-cause death or heart transplantation in heart failure patients.

ROC curve analysis results showed that baseline sST2 has a good predictive value for all-cause death or heart transplantation in patients with heart failure at 3 months, 1 year and 3 years.

In 2015, the sST2 international expert consensus recommended 35 ng/ml as the cutoff value for acute heart failure poor prognosis. The optimal cutoff value obtained in this study was 25 ng/ml. Perhaps for the Chinese population, the actual optimal cutoff value of sST2 for heart failure prognostic evaluation is smaller than the current internationally recommended cutoff value.

Multi-factor Cox regression model risk prediction test results show that the predictive value of baseline sST2 for all-cause death or heart transplantation in patients with heart failure is slightly lower than that of baseline NT-proBNP. The combined use of baseline sST2 on the basis of baseline NT-proBNP can predict heart failure. Failure prognosis assessment provides additional predictive value.
