Case review
"Doctor, look at my child. It has been three months old. The yellowness on his body has not only not diminished, but it has become heavier and heavier, and his stool has also begun to turn white." Holding a 3-month-old baby, an anxious mother, walked into the outpatient clinic of the General Surgery Department (Neonatal Surgery, Oncology Surgery) of Hebei Provincial Children's Hospital.
Since the child entered the clinic, Director Niu Huizhong noticed the appearance of the child. It is understood that after the baby’s skin and sclera became yellow and gradually worsened. Local hospitals conducted drug treatment, and jaundice did not reduce it but became worse. The child's mother said that the baby's stool has turned white these days. While comforting parents' emotions, we judge the child's jaundice and considering that the child may have biliary atresia. After being admitted to the hospital, after ultrasound, laboratory tests and other series of examinations, the patient was treated with drugs first. The child's stool condition did not improve, and biliary atresia was further considered. Prepare and undergo cholangiogram examination before and after surgery to clarify the diagnosis: biliary atresia type III, and undergo Kasai surgery. After the operation, the child's liver function dropped rapidly and his stool turned golden.
Seeing the child whose jaundice has subsided and seeing the child's yellow stool, the parents showed a long-lost smile on their faces.
What is biliary atresia
biliary atresia is a life-threatening liver and gallbladder system disease that occurs in infancy. It is characterized by progressive inflammation and fiber obstruction of intrahepatic and extrahepatic bile ducts, which leads to cholestasis , as well as progressive liver fibrosis and cirrhosis , and liver failure will occur in the late stage. According to statistics, biliary atresia is more common in Asians, and infants are more common than those in boys.
Causes of biliary atresia
The etiology and pathogenesis of biliary atresia are not yet clear, and may be related to congenital biliary phylogenetic abnormalities, perinatal viral infection, and abnormal pancreatic duct confluence.
Disease classification
biliary atresia, currently commonly used is the Gassian classification method. This classification method divides biliary atresia into three basic types:
1 type: common bile duct atresia
https://www. ml1 type II: liver duct atresia
type III: hilar atresia
clinical manifestations
mostly have full term , and there are no abnormalities for more than 1-2 weeks after birth. After physiological jaundice subsides, the skin becomes yellow again, the urine gradually becomes darker, and even becomes thick brown. The stool color gradually becomes lighter, and it becomes light yellow or clay-like white stool.
We can observe the color of the stool through the stool colorimetry card to identify bile duct atresia early. As the liver gradually grows bigger, abdominal bloating and abnormal growth and development occur.
How to treat
biliary tract reconstruction is an effective method to treat biliary atresia. Once biliary atresia is diagnosed, early surgical treatment should be performed. Through Kasai surgery, normal bile drainage is restored, liver function is improved, and long-term survival is achieved. The visit time is late, and the surgical effect may be affected by irreversible cirrhosis.
Expert reminds
Hebei Province and director of the General Surgery Department 1 (Neonatal Surgery, Oncology Surgery) of Hebei Provincial Children's Hospital Niu Huizhong reminds the parents: biliary atresia must be discovered early, early treatment, and early treatment to avoid irreparable consequences due to delaying the condition.
Department introduction ✦
General Surgery Department (Neonatal Surgery, Oncology Surgery) of Hebei Provincial Children's Hospital is a key clinical specialty in Hebei Province and a key development discipline in medicine in Hebei Province. There are currently three wards of general Surgery, intensive care and neonatal ward.
department main features include three branches and majors: pediatric general surgery, pediatric abdominal solid tumor surgery, and pediatric general surgery severe disease, covering various gastrointestinal congenital structural deformities, neonatal surgical diseases, abdominal solid tumors, abdominal trauma, compound injury , acute abdominal disease , etc. In recent years, minimally invasive surgery has become a major feature of the department. Common diseases and emergency departments of general surgery and neonatal surgery include: congenital megacolon , common bile duct cyst, congenital intestinal malrotation , intestinal atresia, rectal anal malformation, congenital hypertrophic pyloric stenosis , appendicitis , intestinal obstruction, peritonitis , Mekel diverticulum, pediatric hernia , and some abdominal solid tumors are equal, and the surgery can be completed by laparoscopy. At present, the early diagnosis and sequential treatment of malignant solid tumors in children's abdominal areas have formed a standardized and effective diagnosis and treatment process.
