In daily life and work, more and more people have shoulder pain and . Many people will think of periarthritis of shoulder as long as they experience shoulder pain and difficulty raising their hands and moving . In fact, they fail the examination and think it is wrong. not all shoulder pain is periarthritis of shoulder.

I have conducted a summary of the examinations and summary of patients with shoulder pain in my clinical work. I found that about 90% of the patients with hemiplegia have shoulder pain . At first, I thought it was because these patients could not move their limbs after they started to develop, and the complications that occurred after lying for a long time - widening of the shoulder space or subluxation.

But this is not the case. As long as hemiplegia patients, whether their shoulder pain or not, they are used to patting the MRI on their shoulders. They will be surprised to find that most patients have partial tendon injuries and effusions in the joints.

So, at this time, you can think about why so many patients have tendon injuries? Fluid accumulation in the joint? Is it really related to hemiplegia? This is a question worth pondering.

For the shoulder joint, it can move in multiple directions. It is a relatively flexible joint. There is no more flexible joint in the joints of the whole body. This is determined by the characteristics of its own joint structure. On the contrary, has poor stability; as we move daily, the shoulder will become more and more unstable without trauma, and gradually damage .

In many patients with shoulder pain, will more or less report supraspinatus tendon damage and effusion in joints, is too easy to damage. Why is this happening with

?

is mainly related to the position of the supraspinatus muscle. It is at the top of our shoulders. mainly stabilizes the shoulder joint and starts raising the hand. From an anatomical point of view, it crosses the lower gap of acromion . Between the bone at the top of the arm and the cervical vertebrae, there is the bone of the coracoid process in front, and the bone of the acromion above, which is surrounded by these structures. As shown in the figure below:

Langularly

Langularly

This tendon just accommodates the supraspinatus tendon and the underacromial bursal. If the gap of becomes narrower, it is particularly easy for you to cause damage to the supraspinatus muscle and bursal to rub against the bones during the process of raising your hand.

It is precisely because of this that among all our sub-healthy people, some people's acromion, this bone is born abnormal. It will be arc-shaped or hook-like, which will have a special impact on the gap under the acromion of . It will be very narrow and easily cause damage to the supraspinatus tendon.

In addition, the most common thing is in life, because you have done a lot of things and don’t pay attention to protecting your shoulder joints. Many people are even busy for work, and they are deformed. They don’t have a good posture while standing or sitting, and they are basically bent down and hunched. This will mean that the muscle strength around your shoulders is unbalanced. Some are weak and some are nervous. In the long run, it will force the bones on the shoulders of to change the position of ; naturally, when you raise your hand, it will gradually impact and friction, causing damage to the supraspinatus tendon.

So, for these two reasons, many people will suffer from shoulder pain. Just pat the shoulder MRI, and one by one, . However, in real life, when people experience shoulder pain, they like to say that it is periarthritis of the shoulder, and they don’t think it is a supraspinatus tendon injury.

How to determine if it is a supraspinatus injury?

Generally speaking, from your own symptoms, the following characteristics will mainly occur:

1. Pain

There will be impacts on the acromion and damage to the supraspinatus tendon. When it is not too serious in the early stage, you will only feel that shoulder soreness and swelling , especially in the morning and evening or when the weather becomes colder; if you are not careful, it will gradually aggravate, and gradually appear at a certain angle of shoulder lifting, which will sting.

2. Difficulties in activity

As the pain occurs, I feel more and more afraid of raising my hand. I always take it for granted to protect this hand when doing things. Some people even do not use this hand to do things because of pain. Slowly, they will have shoulder joint movement disorders, which feels like periarthritis of the shoulder, but it is not.

If you have shoulder pain, you can check for self-check first. Is the acromion impact causing damage to the supraspinatus tendon?

3. Look at the pain range

As long as there is shoulder pain, it is not within the range of the entire time you raise your hand, you must pay attention to whether it will be pain in a certain range? If there is a certain pain range, we need to consider the impact problem we talked about this time. It is more common for pain in raising your hand to the 60°-120° range, and other ranges are fine.

4. Pour water

Hand on the pain side, lift your arm outward, as shown in the picture above, turn your hand over and pour out all the water to see if the shoulder hurts during this process. If there is pain, you need to consider impact or supraspinatus tendon damage. This test is also relatively accurate, you can try it.

Finally, if there are abnormalities in the above symptoms and examinations, it is strongly recommended that you go to the hospital to to pat the MRI on your shoulder to clarify . Other films cannot see this problem.

How to prevent damage to the supraspinatus tendon?

1. Pay attention to life behavioral habits

In daily work, do not sit hunched for a long time, straighten your body, extend your shoulders to both sides, and tighten your scapula is the best state; when doing things, do less movements involving your arms to your chest; pay attention to ball exercises such as swimming and slap, which is a common cause of tendon damage.

2. Strengthen the exercise of shoulder external rotating muscles

as shown in the figure below. Pull a elastic band in your hand, bend your arm 90 degrees, and pull the elastic band to the outside and open it with your hand. It is best to put a soft pad between your elbow and your body. The purpose is to not run to the outside with the elastic band during the pulling process. As long as this is the effect of the exercise will be better. Friends who are fine, do more exercises to help your shoulder health.

What should I do if there is any pain in the event of injury?

The treatment problem is handed over to the doctor in the hospital. There are currently two treatment methods: surgery and conservative treatment.

1. Surgical treatment

If the examination is clear, your acromion bone will be born abnormal. Choose surgical treatment directly, do , acromoplasty , and then go to the rehabilitation department for functional exercise. If the supraspinatus tendon is torn severely in the film, perform arthroscopic repair surgery as soon as possible , and then go to the rehabilitation department for rehabilitation function exercises.

2. Conservative treatment

Except for the above situations, you can choose to go to the rehabilitation medicine department for conservative treatment, which can generally achieve the effect of analgesic or improving function.

For patients with partial tendon injuries, platelet-rich plasma factor can be injected into the shoulder joint (a certain amount of your blood is extracted, and the plasma factor is left after centrifugation of the machine) to facilitate the repair of tendon injuries and is very effective in the treatment.

At the same time, you must also find out the imbalance of your shoulder mechanical around the evaluation of the therapist, and then give targeted adjustments to the treatment. This step is very critical; if it is not done, even if your damaged tendon is repaired through plasma factor injection, you will continue to be damaged in the future when your mechanical imbalance is not balanced. Therefore, exercise adjustments under the therapist are the foundation of treatment.

Summary

The incidence of shoulder pain is very high, and basically everyone may experience it, but Everyone should pay attention to the supraspinatus tendon injury accounts for about 85% of the causes of shoulder pain, which is not what everyone thinks of periarthritis.
