It is said that the parents who are following are all very cute and cute!
All these 21 situations can be vaccinated, don’t be afraid!
The audience of this article is pediatricians with a desperate spirit and parents with a research spirit. The goal of this article is to be a little expert after reading it.
Introduction:
In fact, I originally wanted to get a title party, for example, you cannot get vaccinated in these situations, remember. Through this scary topic, you can attract readers and then tell everyone not to be afraid, such as jaundice, eczema, and congenital heart disease, you can get vaccinated.
Haha, but then I thought, we are just hard-core popular science, we just don’t want parents to be afraid. It’s enough to share the correct popular science with everyone, so the question is now.
So today I will explain the situation that I dare not get vaccinated or that I was unclear about it. For example, how to get vaccinated after using globules and hormones, how to get vaccinated children with febrile convulsions, epilepsy, and cerebral palsy, can I still get vaccinated with BCG vaccine after using hepatitis B immunoglobulin, etc.
So, let’s start your journey of peace of mind.
P.S. This article is a brief summary of the "Expert Consensus on Vaccination of Children with Special Health Status". There are 21 situations in the consensus, and 13 common situations are selected in this article.

1.What situations do you think you cannot get vaccinated?
Before, due to the unclear domestic guidelines and consensus, it is unclear whether children with fever convulsions, children with asthma, children with immunodeficiency, children with food allergies (egg, milk), children with epilepsy, children with perianal abscesses, children with eczema, children with jaundice, and so on, whether they can get vaccinated and when they can get vaccinated.
So, our doctors have no idea what to do with these babies. Use it according to foreign regulations. Although there is no error, no one can bear the responsibility for something. It is better to have less trouble than more. Basically, these babies are vaccinated very late and even cannot get vaccinated.
2. Do we have a guide now?
actually has no, but now there is an expert consensus, which is the "Expert Consensus on Vaccination of Children with Special Health Status". There are currently 21 articles published in the "Chinese Journal of Practical Pediatrics" in October 2018 and May 2019.
Some people will definitely not read the article, and will directly say that the level of evidence of expert consensus is not high and cannot be used as clinical reference; in addition, some people will definitely say that it is designated by Chinese experts, so it is better not to read it. Haha, you can take a break. I have read the consensus of
. The references to the consensus articles are very professional and solid, including WHO, American CDC, and clinical trial data from major authoritative journals. The final conclusion is credible. And some recommendations are more relaxed than those given by the United States, and they are very courageous and responsible.
At the same time, these conclusions do not involve the interests of the manufacturer, and there is no recommended vaccine in it. Therefore, everyone can use it with confidence, especially front-line clinical vaccination personnel.
gives a thumbs up to this expert consensus. Please continue to read the specific content.
3. How to get vaccinated for children with food allergies?
• Children with food allergies can receive normal vaccination according to the immunization procedure.
• Children with a history of severe systemic allergic reactions in eggs should receive influenza vaccine under the supervision of medical institutions.
•In acute reaction periods of food allergies (such as concurrent asthma, urticaria, etc.) or abnormal skin at the vaccination site (eczema, atopic dermatitis, etc.), vaccination should be temporarily postponed. The second half of this article means that if there is no eczema at the vaccination site, you can get vaccinated.
•People allergic to eggs are contraindicated for getting vaccinated with yellow fever. Basically, we can't use this if we don't go abroad to Africa.
4. How to get vaccinated for children with congenital heart disease (CHD)?
can be inoculated:
•Growth and development well, no clinical symptoms, and no abnormal heart function [such as left ventricular ejaculation fraction (LVEF) ≥60%];
•College heart disease in children with congenital heart disease, after interventional treatment, no abnormal heart function was checked;
•College heart disease in children with congenital heart disease, 3 months after surgery, no abnormal heart function was checked.
Responsible vaccination:
•Children with complications such as cardiac insufficiency and severe pulmonary hypertension;
•Children with complex cyanosis (cyanosis) type CHD, who require multiple hospitalization operations;
•Other situations that require specialist evaluation, such as immunodeficiency, infection, severe malnutrition, and use of immunosuppressants.
5.How to get vaccinated for children with eczema?
can be vaccinated with various vaccines and avoid eczema areas. If the vaccination site is full of eczema, vaccination must be postponed.
6. How to get vaccinated for children with febrile convulsions (FS)?
First talk about the definition and characteristics of fever convulsions:
• Convulsion attacks occur during the first fever (anal temperature ≥ 38.5℃, axillary temperature ≥ 38℃), there is no evidence of central nervous system infection and other causes of convulsions, and there is no history of fever convulsions.
•The peak incidence is between 12 and 18 months, and the incidence rate in men is slightly higher than that in women. Most of them have benign outcomes, and fever convulsions no longer occur as they age.
•Simple FS is manifested as a total attack, with a duration of convulsions of <15>
•Complexity FS is manifested as a focal or a total attack, with a duration of convulsions of ≥15 minutes, or an attack ≥2 times during one fever course.
About the content of fever convulsions, you can read the article: Reject dogma and let you know what to do when your baby has fever and fever convulsions! Part 2.
The following is the conclusion, which is very good and is very bold:
For simple FS, or non-frequency febrile convulsions (attacks <3>
7. How to get vaccinated for children with epilepsy?
Let’s first talk about what epilepsy is: epilepsy is a chronic recurrent transient brain dysfunction syndrome characterized by abnormal discharge of brain neurons.
Vaccination for children with epilepsy helps prevent infectious diseases and prevents further aggravation of brain damage. At the same time, the adverse reactions caused by children with epilepsy have not increased, so there are no contraindications for children with epilepsy to get vaccinated.
Therefore, the conclusion given by this consensus is that patients with epilepsy who have not had seizures for 6 months or more (epilepsy has been controlled) can receive all vaccines regardless of whether they take antiepileptic drugs. Everyone, it is any vaccine, including various brain-containing vaccines, such as J-encephalitis, meningitis, polio vaccines, etc.
•People with a family history of epilepsy can get vaccinated.
8. How to get vaccinated for children with cerebral palsy?
Cerebral palsy is cerebral palsy, a group of persistent central motor and posture development disorders and limited movement syndromes. Have you seen it? Cerebral palsy is a problem of exercise and posture, not a child's stupidity, although children with cerebral palsy are usually accompanied by cognitive and communication disorders.
Cerebral palsy is non-progressive, that is, brain injury is static, so it is safe to vaccinate children with cerebral palsy. At the same time, among patients with cerebral palsy, 20% have swallowing disorders, which will cause repeated aspiration, and respiratory muscle coordination disorders, increasing the chance of lung infection.
Therefore, the immune protection brought by vaccination will benefit children with cerebral palsy and can reduce the occurrence of preventable diseases by vaccines.
Therefore, the conclusion of this consensus is that children with cerebral palsy can receive any vaccine according to the immunization procedure.
9. How to get vaccinated for children with perianal abscess?
Regarding what is a perianal abscess, you can read the article: What does a perianal abscess look like? Why are boys more common? Is conservative or surgery better? Can I get vaccinated?
Currently, research suggests that perianal abscess is one of the high-risk factors for the occurrence of vaccination after oral live attenuated polio vaccine (OPV), and the use of inactivated injectable polio vaccine (IPV) does not have this risk. Therefore, it is recommended:
polio vaccine basic immunization (2 months, 3 months, 4 months) use IPV, and strengthen immunization after recovery (at 4 years old) can be vaccinated with IPV or OPV.
10. How to get vaccinated for children with jaundice?
can be vaccinated:
• Children with physiological jaundice and breast milk jaundice are in good health and can be vaccinated according to the immunization procedure.
•Children with pathological jaundice have stable vital signs and can receive hepatitis B vaccine normally.
Response to vaccination:
Children with pathological jaundice need to find out the cause in time and other vaccinations are temporarily postponed. It is recommended to go to a specialist outpatient clinic for treatment.
In fact, this can be remembered as long as the doctor still says that your child’s jaundice is not required to be treated with blue light and then get vaccinated.
11. How do children using gamma globulin get vaccinated?
Whether users of Globulin C (IVIG for short) can be vaccinated is mainly related to whether Globulin C contains antibodies to the pathogen prevented by the vaccine.
If the content of a specific antibody in the sphere of C, the greater the impact on its corresponding vaccine. Because gamma C globulin (bulb) is extracted from human blood, and people generally undergo artificial immunization or natural infection of measles, bulb C contains a certain amount of measles antibodies.
●So, if a large dose (2 g/kg) of gamma globulin is used, you need to wait 8-9 months before you can get a vaccine containing measles components after the measles antibodies in the gamma.
●For vaccines that do not contain measles, they will be vaccinated at will, and there will be no effect (see the comments at the bottom of this article). For example, other live vaccines: BCG, JE vaccine, etc., will not affect them.
can also be introduced from above. After using hepatitis B immunoglobulin, you can use BCG directly.
But this recommendation is a bit simple. The following is a recommendation from the US CDC. They gave it very detailed and listed hepatitis B immunoglobulin, etc., so, please look at the picture (click to see the larger picture):
Note: Although it says that vaccines without numb can be vaccinated at will, the state's management of vaccines (2016) stipulates that those who inject immunoglobulin should receive numb vaccines, encephalitis B vaccines, and live attenuated hepatitis A vaccines at a distance of ≥3 months, so you have to wait at least 3 months. Avoid using immunoglobulin within 2 weeks after receiving the leprosy vaccine.
12. How do children who use hormones get vaccinated?
hormone can inhibit the proliferation and function of immune response-related cells (T cells, B cells, macrophages, etc.). Therefore, it was previously believed that vaccination will be poor when using hormones. However, the trials have shown that the use of hormones has no effect on the immune effect of the vaccine, and there are no adverse reactions. Therefore, this consensus recommends:
●While using hormones, you can receive any inactivated vaccine.
●When using hormones, live attenuated vaccines need to be temporarily postponed. Large doses of glucocorticoid [prednisone ≥20 mg/d or >2 mg (/kg·d)] can be vaccinated with live attenuated vaccine 1 month after the end of treatment.
Why should live attenuated vaccines be slow? Because the live vaccine has the ability to infect, it is just a toxic weakness. After using up hormones, the human immunity is limited, and it may not be able to withstand this vaccine, resulting in infection.
13. How to get vaccinated with infectious diseases?
is how to get vaccinated when a child catches a cold, fever, diarrhea, pneumonia and other diseases. The consensus is quite complicated, but to put it simply, it is:
●Mini diseases such as colds and diarrhea can be vaccinated as long as they don’t have fever.
●Do not take live attenuated vaccines orally, such as rotoform, polio vaccine (OPV).
●Severe diseases, such as pneumonia, meningitis, etc., can only be vaccinated after the disease is completely cured.
Haha, there are currently 21 articles in this consensus, and there are also children with asthma, children with immunodeficiency, children with allergic purpura, children with autoimmune diseases, children with nephritis, etc. How to get vaccinated? I chose the easiest situations for everyone to see. If you are also interested in other situations, you can read the original text.
