Faced with the "ultimate challenge" of rare and huge tumors, the team of 10 major majors in Wuhan University People's Hospital "formed a group" to deal with it calmly. Finally, after a 10-hour systematic operation, it was difficult to "pick the melons" successfully.

A recurrent thymoma with a diameter of more than 20 cm and weighing 3 catties, occupying the entire right chest cavity, causing Ms. Liu's life to light up a red light. Faced with the "ultimate challenge" of rare and huge tumors, the 10 university expert team of Wuhan University People's Hospital ( Hubei Provincial People's Hospital ) responded calmly. After a 10-hour systematic operation, it was difficult to "pick the melons" successfully. On October 10, Ms. Liu, who was reborn, recovered and was discharged from the hospital.

15 years ago, Ms. Liu unfortunately suffered from anterior mediastinal B2 thymoma. After undergoing transsternal mediastinal tumor resection and postoperative chemotherapy, her condition was stable for a while, and she has not had regular reviews for many years since then. Half a year ago, Ms. Liu was diagnosed with relapse of thymoma after suffering from chest tightness and pain again. Professor Geng Qing , Director of the Department of Thoracic Surgery at Wuhan University People's Hospital, led a detailed examination and found that the thymus tumor removed 15 years ago not only relapsed again, but also quietly grew into the size of a "winter melon", invading the right chest wall, and at the same time compressing her right lung, heart, diaphragm, trachea, liver, and right kidney. If it is not "removed" in time, it will directly endanger life.

But such a rare huge thymoma , coupled with the influence of radiotherapy and chemotherapy, it is never easy to reexamine again. In order to improve the treatment plan, Professor Geng Qing's team organized MDT consultations in 10 disciplines including the hospital, including anesthesia department, critical care medicine department, , radiological interventional department, hepatobiliary surgery, urology department, respiratory medicine department, oncology department, , radiotherapy department, and pathology department. After repeated discussions, experts believed that chemotherapy for the huge thymoma was not effective after recurrence, and tumor compression led to a narrow operable space and serious stickiness, and the heart, large blood vessels and nerves were distributed around the tumor, which made the surgery complicated and extremely risky.

After repeated communication with the patient and his family, the medical team finally decided that the thoracic surgery expert team would perform the resection surgery, and the hepatobiliary and urology expert teams would be ready for intraoperative assistance at any time.

Before the operation, Professor Geng Qing’s team carefully analyzed the possible situations and formulated careful response and handling strategies. The experts from the preoperative interventional department also performed branch vascular embolism of tumor supply to reduce intraoperative bleeding; the anesthesia department evaluated the risk of intubation and formulated individual anesthesia plans to prevent tumor pressure, trachea, heart, etc. during the anesthesia process from affecting respiratory and circulation functions.

After sufficient preoperative preparation, on September 26, the multi-disciplinary diagnosis and treatment team performed resection surgery on the patient. The team of Professor Xia Zhongyuan of the Department of Anesthesiology escorted the entire process. Professor Geng Qing's team carried out a huge and complex systematic surgery, "High-opening thoracic mediastinal tumor resection + partial pulmonary resection + diaphragm reconstruction + chest wall tumor resection + thoracic reconstruction".

During the operation, experts found that the patient had serious adhesions in the thoracic cavity. The huge tumor not only clings to the entire chest cavity, compressing the entire right lung tissue, but also invades the diaphragm and compressing the abdominal liver, kidney and other trachea. The heart, large blood vessels and trachea have been pushed to the left. Like " ant chewing bones", the surgical team overcame many dangerous situations such as tumor rupture and bleeding, diaphragm invasion and defects. After 10 hours of hard work, the tumors as big as "winter melon" were broken down into small pieces and then stripped off accurately.

After careful care by the medical team, Ms. Liu recovered smoothly after surgery and was finally discharged from the hospital on October 10.
