On April 9, 2021, a explosion accident occurred at the Qizi Liquefied Gas Station of Shaofeng Property Company of Hunan Shaofeng Cement Group, causing 2 deaths and 3 injuries.

(I) Direct cause of the accident

illegal operation. Before inflating the 2# cylinder, Li Moushu did not conduct a comprehensive inspection of the 2# cylinder in accordance with the "Regulations on Filling Liquefied Gas Cylinders" (GB/T 14193-2009) and the "Safe Operating Regulations for Filling Gas Cylinders" of Qizi Liquefied Gas Station and took corresponding treatment measures. He did not promptly discover and eliminate mixed gas containing oxygen in the cylinder. The liquefied petroleum gas charged from the gas phase valve rubs against the mixed gas containing oxygen in the bottle and causes static electricity, causing the mixed gas in the cylinder to explode and the bottle body to explode. The explosion debris and shock waves of the cylinder caused Li Moushu to die; the high-temperature gas generated by the explosion of the cylinder ignites the liquefied petroleum gas leaked in the filling pipeline cut by the residue, causing a large fire and burns nearby Chen Moufang to death.

(II) Indirect cause of the accident

1. Problems in Qizi liquefied gas station

(1) failed to promptly correct the behavior of the operators' violations of operating procedures . (2) Qizi Liquefied Gas Station unauthorized filling gas for non-owned gas cylinders. The 50kg cylinder that exploded on the filling position 2# in the accident was stored in Peng Mouyan in 2014, and it was not owned by Qizi liquefied gas station. On April 9, 2021, the cylinder was collected by Li Moushu and Gong, a staff member of Qizi Liquefied Gas Station, and exploded during the filling of Qizi Liquefied Gas Station.

2. Problems with Shaofeng Property Company of Hunan Shaofeng Cement Group

Hunan Shaofeng Cement Group Shaofeng Property Company has signed a leasing contract with Liu Mouping since 2005.
