“10·24” major fire accident investigation and handling opinions” (Handan Emergency Submission [2020] No. 14) was received. After research, the approval is as follows:
1. Agree to the determination of the nature of the "10.24" fire accident in Hebei Xinghua Iron and Steel Co., Ltd.
2. Agree to the handling opinions of the accident investigation team on the units and responsible persons of the accident
(I) Transfer to judicial authorities to take criminal compulsory measures (7 people).
1. Liu Haijiang, the masses, the ring cooling worker of the sintering workshop of the steel plant of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for the operation of the ring cooling machine. During the implementation of the production suspension, no excessive amount of sintered ore sintered raw materials were found to enter the ring cooler, and corresponding control measures such as speed reduction were not taken; if the sintered ore in the ring cooler was not emptied, the ring cooler was turned off 18 minutes in advance, and other measures such as closing the ring cooler were not taken, resulting in the sintered ore cooling not meeting the standards, and a large amount of sintered ore red material was generated, causing a fire, and they were directly responsible for the accident. 2. Guo Rijie, the masses, the team leader of Class B in the sintering workshop of the Iron Smelting Workshop of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for the overall work of this team. The training and education of the technical operating procedures and emergency response plans of this team were not in place; the illegal operation of this team was not discovered and stopped in a timely manner; when organizing and directing fire extinguishing, the team members were not arranged to pay attention to the development trend of the fire, the team members were not required to wear protective supplies and pay attention to safety, and the team members were not organized to evacuate in a timely manner, causing casualties to fire extinguishers, and are directly responsible for the accident. Suspected of major liability accidents, he was criminally detained by the public security organs on October 31, 2019.
3.Lu Yanjun, the masses, the leader of the sintering machine fire watch group of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for implementing the operation of the sintering machine system. When the sintering machine tail material is not pushed empty, the main exhaust fan of the sintering machine is turned off 4 minutes in advance, causing excessive sintering ore clipped raw tail material to continue to burn in the ring cooler, which relatively extends the cooling time of the sintering ore and increases the red material generation factor, and is directly responsible for the accident. Suspected of major liability accidents, he was criminally detained by the public security organs on October 31, 2019.
4. Li Jinbing, the masses, the leader on duty in the sintering workshop of the Hebei Xinghua Iron and Steel Co., Ltd. on the day of the accident. During the process of implementing the sintering machine shutdown, the site was not inspected as required, and employees were not promptly discovered and stopped from illegal operations and risky firefighting behaviors; when the fire situation expanded, the initial firefighting personnel were not evacuated in a timely manner, and they were directly responsible for the accident. He was suspected of major responsible accidents and was criminally detained by the public security organs on November 9, 2019.
5. Wang Yanjie, a member of the Communist Party of China, director of the Iron Smelting Plant of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for the comprehensive work of the Iron Smelting Plant.When the sintering workshop implements the closure of finished belts and the corridor sealing transformation, the organization of risk identification and control is not in place; the organization of safety education and training of iron smelting plants, emergency rescue drills, and hidden danger detection is not in place; as the leader of the leadership group for the iron smelting plant suspension organization organization and structure of the iron smelting plant failure to organize and direct the suspension of production work in accordance with the relevant regulations of the enterprise, and the organization and leadership of the smelting machine suspension work is not in place, and he bears direct leadership responsibility for the accident. Suspected of major liability accidents, he was taken by the public security organs on February 20, 2020 to bail pending trial.
6. Zhang Jianjun, a member of the Communist Party of China, assistant to the general manager of Hebei Xinghua Iron and Steel Co., Ltd., is in charge of the company's production safety work, and the company's on duty leader on the day of the accident. When the finished belt closure and corridor sealing transformation of the sintering workshop, risk identification and control were not organized; safety education and training, emergency rescue drills, and hidden danger investigation and rectification of the unit were not in place; failure to go to the production suspension site for inspection as required by the duty system, and failure to promptly discover and stop employees' illegal operations and risky firefighting behaviors; failure to organize the evacuation of initial firefighters when the fire situation expands; failure to report the accident situation in a timely and truthful manner in accordance with relevant regulations, and bear direct leadership responsibility for the accident. He was suspected of major responsible accidents and was criminally detained by the public security organs on November 9, 2019.
7. Fu Xingang, a member of the Communist Party of China, general manager of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for the company's overall work. At the company meeting, the iron smelting plant director Wang Yanjie was arranged to be responsible for the suspension of the sintering workshop. The work arrangements were not meticulous or strict; the company's production safety work was not in place, and the hidden dangers of production safety accidents were not eliminated in a timely manner; the company's safety education and training, emergency rescue drills, and hidden danger investigation and rectification were not in place; the company's accident situation was not reported in a timely and truthful manner in accordance with relevant regulations, and the injured person did not report truthfully in accordance with regulations, and the main leadership responsibility for the accident was borne. He was suspected of major responsible accidents and was criminally detained by the public security organs on November 9, 2019. If a person suspected of committing crimes is a member of the Communist Party of China after the judicial authorities handles it, the discipline inspection and supervision organs of Wu'an City shall promptly impose corresponding party discipline sanctions in accordance with relevant regulations.
(II) Responsible persons (1 person) for internal processing of the enterprise.
Tian Liang, the masses, deputy director (the director vacant) of the sintering workshop of the Iron Smelting Workshop of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for the comprehensive work of the sintering workshop. 3. Wei Haiming, a member of the Communist Party of China, director of the Security Management Office of Wu'an Metallurgical Mining Group Company, is responsible for supervising subordinate subsidiaries to perform their duties of supervising corporate safety. 6. Fan Xiaozhou, a member of the Communist Party of China, head of the Energy Section of the Wu'an Science and Technology and Industrial Information Bureau, and the person in charge of grid supervision and subcontracting for steel enterprises, is responsible for carrying out safety supervision and inspection. 8. Du Zhengping, a member of the Communist Party of China, Secretary-General of the Industrial Economic Federation of Wu'an Science and Technology and Industrial Information Technology Bureau, and Deputy Grid Director of Grid Supervision of Steel Enterprises, is responsible for deploying and organizing all work on safe production. He bears the main leadership responsibility for the accident and is given a demerit in administrative affairs.
9. Gu Gailan, a member of the Communist Party of China, director of the Wu'an Science and Technology and Industrial Information Technology Bureau, and the grid director of the grid supervision of steel enterprises, is responsible for the comprehensive work of safe production. The awareness of production safety is not strong, the superiors' requirements for production safety are not implemented in place, and they are ineffective in leading the work of production safety. He bears important leadership responsibilities for the accident and is given a government warning.
10. Zhao Lijie, a member of the Communist Party of China, a staff member of the 11th Group of the Law Enforcement Squadron of Wu'an Emergency Management Bureau, is stationed in Hebei Xinghua Iron and Steel Co., Ltd. during the period of production suspension and restrictions, and is responsible for supervising the entire process of production suspension and restrictions. There is insufficient supervision of the safety education and training of Hebei Xinghua Iron and Steel Co., Ltd. for production suspension, and there is a lack of supervision and management of the production suspension site. He bears direct leadership responsibility for the accident and is given a serious warning within the party.
11. Liu Zhiguo, a member of the Communist Party of China, and the head of the Metallurgical Building Materials Office of Wu'an Emergency Management Bureau, is responsible for implementing comprehensive supervision and management of metallurgical building materials enterprises and supervising and inspecting the implementation of production safety laws and regulations of metallurgical building materials enterprises. Supervision of the safety training and emergency plan drills for Hebei Xinghua Iron and Steel Co., Ltd. has not been in place. He bears the main leadership responsibility for the accident and is given a warning within the party.
12. Song Hongshun, a member of the Communist Party of China, chief clerk of Wu'an Emergency Management Bureau, is in charge of the Metallurgical Building Materials Office. The leaders of the department in charge were not effective and the inspection of Hebei Xinghua Iron and Steel Co., Ltd. was not in place. He bears important leadership responsibilities for the accident and is given a government warning.
13. Liu Bingzhao, a member of the Communist Party of China, head of the Atmospheric Section of the Wu'an Branch of the Handan Ecological Environment Bureau, is responsible for managing and guiding the prevention of production safety accidents during the suspension and restriction of Hebei Xinghua Iron and Steel Co., Ltd.He has a weak awareness of production safety and has not strictly followed the requirements of the "Notice on Doing a Good Job in Production and Suspension of Production and Safety Accidents in Production and Operation Units" (Ji'an Supervision [2018] No. 13), supervising and detecting hidden dangers for the suspension of production and restrictions of Hebei Xinghua Iron and Steel Co., Ltd. He is responsible for supervision of the accident and is given a demerit in administrative affairs.
14. Zhang Jixing, a member of the Communist Party of China, deputy director of the Wu'an Branch of the Handan Ecological Environment Bureau, is in charge of the Atmospheric Department. He has a weak awareness of production safety, has poor leadership over the departments in charge, and has not arranged and deployed the work of shutting down production and restricting production safety. He bears the main leadership responsibility for the accident and is given a government warning.
15. Li Weifeng, a member of the Communist Party of China, was then Secretary-General of the Wu'an Municipal Government (currently member of the Standing Committee of the Wu'an Municipal Party Committee and Minister of the United Front Work Department), and was in charge of the Wu'an Science and Technology and Industrial Information Technology Bureau and the Wu'an Metallurgical Mining Group Company. The arrangements and deployments for safe production work are not in place, and the leadership of Wu'an Science and Technology and Industrial Information Technology Bureau and Wu'an Metallurgical Mining Group Corporation is not effective. He bears leadership responsibility for the accident and gives a warning.
(IV) The responsible persons (7 people) who impose administrative penalties.
1. Li Yanrong, a member of the Communist Party of China, head of the environmental protection section of the Iron Smelting Plant of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for the environmental protection work of the iron smelting plant. The "one post, two responsibilities" requirement was not implemented. When the sintering workshop implemented the finished belt closure and corridor sealing renovation, the risk identification and control were not carried out in combination with the workshop and the factory's safety management personnel, and they were mainly responsible for the accident. Hebei Xinghua Iron and Steel Co., Ltd. will be dismissed, and the Handan Emergency Management Bureau will impose an administrative penalty of RMB 5,000 in accordance with Article 79, Paragraph (I) of the "H Hebei Province Production Safety Regulations ".

4. Yu Jinjiang, the masses, deputy director of the Iron Smelting Plant of Hebei Xinghua Iron and Steel Co., Ltd., is in charge of the safety production work of our factory. Hebei Xinghua Iron and Steel Co., Ltd. will be dismissed, and the Handan Emergency Management Bureau will impose an administrative penalty of RMB 8,000 in accordance with Article 79, Paragraph (I) of the "Hebei Province Production Safety Regulations".
5. Liu Haihua, Qunzhong, Director of the Energy and Environmental Protection Department of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for the company's energy and environmental protection work. The "one post, two responsibilities" requirement was not implemented. When the finished belt closure and corridor seal renovation was implemented in the sintering workshop, the risk identification and control was not carried out in conjunction with the company's safety department and relevant personnel of the iron smelting plant, and they were mainly responsible for the accident.Hebei Xinghua Iron and Steel Co., Ltd. will be dismissed, and the Handan Emergency Management Bureau will impose an administrative penalty of RMB 9,000 in accordance with the provisions of Article 79, Paragraph (I) of the "Hebei Province Production Safety Regulations".
6.Sun Junsong, the masses, director of the Safety Department of Hebei Xinghua Iron and Steel Co., Ltd., is responsible for the company's safety production management. When the finished belt closure and corridor sealing renovation was implemented in the sintering workshop, risk identification and control were not organized; the company's safety education and training, emergency rescue drills, and hidden danger investigation and rectification were not in place, and they were mainly responsible for the accident. Hebei Xinghua Iron and Steel Co., Ltd. will be dismissed, and the Handan Emergency Management Bureau will impose an administrative penalty of RMB 9,000 in accordance with the provisions of Article 79, Paragraph (I) of the "Hebei Province Production Safety Regulations".
7. According to Article 93 of the "Production Safety Law of the People's Republic of China", the Handan Emergency Management Bureau will revoke the safety certificate of Fu Xingang, general manager of Hebei Xinghua Iron and Steel Co., Ltd. and Zhang Jianjun, assistant general manager, in accordance with the law.
(V) The responsible unit for imposing administrative penalties.
1. Hebei Xinghua Iron and Steel Co., Ltd. has not fully implemented the main responsibility for the enterprise to produce safety, has not promptly discovered and rectified the potential risks of the accident, and has not implemented safety measures during the production suspension process, and is responsible for the accident, and has reported late or falsely reported the accident (after the accident, the enterprise did not report the injured person). According to the provisions of Article 15, paragraph 2 of the "Production Safety Accident Penalty Regulations (Trial)" of the former State Administration for Safety Supervision, the company will be fined RMB 1 million; according to the provisions of Article 36, item (1) of the "Production Safety Accident Reporting and Investigation and Handling Regulations" (State Council Order No. 493), the company will be fined RMB 1.7 million, and the Handan Emergency Management Bureau will be fined RMB 2.7 million for the above two illegal acts. In accordance with the provisions of Article 4 (2) of the Provincial Safety Committee's "Notice on Issuing the Interim Measures for the Management of the "Blacklist" of Production and Operation Units in Hebei Province" (Jii Safety Committee [2016] No. 1), the enterprise is included in the "Blacklist" of Hebei Province's "Blacklist" of Production and Operation Units in Hebei Province.
2. According to the provisions of Article 28, item (2) and item (3) of the Hebei Provincial Emergency Management Department 4 "Hebei Provincial Enterprise Safety Standardization Work Management Measures" (Hebei Emergency Political and Law [2019] No. 100), the Handan Municipal Emergency Management Bureau shall report to the Provincial Emergency Management Department in accordance with the procedures to revoke the second-level safety standardization enterprise level of Hebei Xinghua Iron and Steel Co., Ltd.
(VI) Handling of local governments and relevant regulatory departments.
1. Instructed Wu'an Metallurgical Mining Group Co., Ltd., Wu'an Science and Technology and Industrial Information Technology Bureau, Wu'an Branch of Handan Ecological Environment Bureau, and Wu'an Emergency Management Bureau to make in-depth written inspections to the Wu'an Municipal Party Committee and Municipal Government.
2. Instructed the Wu'an Municipal Party Committee and Municipal Government to make a profound written inspection to the Handan Municipal Party Committee and Municipal Government.
3. Strictly carry out handling and implementation work.
