TIMI risk score is currently a commonly used risk stratified scoring system for patients with acute myocardial infarction. Studies have shown that TIMI risk score has high predictive value for the occurrence of major adverse cardiovascular events in patients with acute coronary syndrome.
Recently, a study published by Fuwai Hospital Yu Mengyue et al. showed that TIMI risk score can also predict the long-term prognosis of patients with coronary non-obstructive myocardial infarction.
This study retrospectively included 461 patients who underwent coronary angiography at Fuwai Hospital from January 2015 to December 2019 and diagnosed with ST-segment elevated coronary artery non-obstructive myocardial infarction. According to the TIMI risk score, 157 were assigned to the low-risk group (≤ 3 points), and the remaining 304 were assigned to the medium- and high-risk group (> 3 points). The median follow-up of
for 41.7 months showed that the incidence of major adverse cardiovascular events in the low-risk group was significantly lower than that in the medium- and high-risk group, at 8.9% and 18.1%, respectively.
Cox regression analysis showed that low risk of TIMI risk score was independently associated with a lower risk of major adverse cardiovascular events (HR=0.468).
ROCh curve analysis suggests that TIMI risk score has certain prognostic predictive value for patients with ST-segment elevated non-obstructive MI (AUC is 0.679).
The author said that in the future research will expand the sample size and combine the TIMI risk score and other cardiovascular risk factors to build a new model that is more suitable for cardiovascular risk assessment in Chinese people. It will include special mechanisms of coronary non-obstructive MI infarction into the evaluation project, such as coronary microcirculation function, further improve the long-term prognostic predictive value of ST-segment elevated coronary non-obstructive MI infarction.
TIMI risk score consists of the following 8 items: (1) Whether the age is between 65 and 74 years old or ≥75 years old; (2) Whether systolic blood pressure is 100 mmHg; (3) Whether heart rate is 100 times/min; (4) Whether diabetes , hypertension , and angina pectoris; (5) Killip grade ≥2; (6) Whether the weight is 67 kg; (7) Whether there is anterior wall myocardial infarction or left bundle branch block in the electrocardiogram; (8) Whether the time for treatment was 4 hours.
