2. On May 10, a personal injury and death occurred in a Lincang Town Kang Power Supply Bureau of Yunnan Power Grid Co., Ltd., with 2 people killed and 1 person seriously injured. An operator of the Lincang Town Kang Power Supply Bureau of China Southern Power Grid Yunnan Power Grid Co., Ltd., was dealing with the special transformer energy meter of the 10 kV umbrella line branch 4 Squadron, and the customer's property rights high-voltage metering box located above the fault meter suddenly exploded, killing 2 people and seriously injuring 1 person.
3. On May 11, a personal injury and death occurred in the Shaoguan Power Supply Bureau of Guangdong Power Grid Co., Ltd. and 2 people died. When the construction of the 10 kV line project of the Shaoguan Power Supply Bureau of Guangdong Power Grid Co., Ltd. of Ruyuan Yao Autonomous County Yinquan Power Industry Co., Ltd. was in the construction of the 10 kV line project of the Shaoguan Power Supply Bureau of China Southern Power Grid Guangdong Power Grid Co., Ltd., the construction personnel did not set anti-wear protection on the crossed low-voltage wires during the tightening process, causing damage to the insulation of the low-voltage line below, causing electric shock accidents and causing two deaths.
4. On May 11, a personal injury and death occurred in the Guojiawan Power Plant in Fugu County, Shaanxi Province, and one person died. During the ultra-low emission transformation project of the Guojiawan Power Plant of Shenhua Shendong Electric Power Co., Ltd., which belongs to the State Energy Investment Group Co., Ltd., during the construction of the small ash hopper at the lower ash hopper of the electrical dust collector of Unit 1, the hanging ears rooted at the upper ash hopper were torn, and the small ash hopper flipped and fell, cutting off the safety belt of the operator, causing two operators to fall from about 32 meters to the 26-meter platform, causing one death.
5. On May 12, a personal injury and death occurred in the Baozishi Hydropower Plant of Jiangxi Ganneng Co., Ltd., and one person died. The maintenance personnel of the maintenance unit Xiushui County Xinghong Construction Decoration Engineering Co., Ltd. fell from the roof insulation layer of the dam water discharge unit of Jiangxi Ganneng Co., Ltd., causing one person to die.
6. On May 20, Sanxia New Energy Pingquan Power Generation Co., Ltd. suffered a personal injury and death, and one person was seriously injured. During the process of handling the tripping fault of Line 3 of the on-site power collection line, the operators encountered an electric shock in the live area of the 35 kV switch cabinet corresponding to the tripping of Line 3, and caused one person to be seriously injured when they checked within the interval of the 35 kV switch cabinet corresponding to the tripping of Line 3.

1. Brief introduction:
On May 9, Wang Moumou, the power supply service team of the Nanshan Power Supply Station of Kang Power Supply Bureau of Lincang Town, found that the power of the special user Lincang Fourth Squadron of the Armed Police was abnormal in April. After inquiring that there was no significant change in the user load in April, and initially suspected that the power meter was abnormal. So Li Moumou (the power supply service staff member) and he went to inspect the special variable metering device on May 10.
At 16:17 on May 10, Wang Moumou filled out the low-voltage reporting form in the 4A-Production Management System-Fault Repair Module.
At 16:19 on May 10, the allocation center will dispatch workers after receiving the ticket.
At 16:24 on May 10, Wang Moumou received the order, and then Wang Moumou, Li Moumou, Zhou Moumou (meter reader) and three others set off from the power supply station.

17:12, after reviewing the operating documents by the supervisor of the station (the safety supervisor of the power supply station stationed by the Zhenkang Power Supply Bureau), the on-site personnel agreed to carry out their work.
17 at 24:00, the protection action of the 110kV white rock transformer 10kV nitroum line overcurrent section I tripped, the reclosing failed, and the fault showed a three-phase fault.
17 at 27:00, a passerby found that the 10kV umbrella line #2 pole 10kV combined transformer exploded and caught fire. Someone was injured at the scene and called 120 emergency number.
17 at 40, the 120 ambulance arrived.
17:45, emergency personnel from Zhenkang County People's Hospital determined that Wang and Zhou were dead on the spot and sent Li to the hospital for treatment.
At present, the injured have been transferred to Kunming Hospital for treatment.
2. Equipment failure
Model: JLSW3-10 (oil immersion), Zhejiang South High and Low Voltage Switch Factory, put into production on July 24, 2018. After on-site inspection and confirmation, it was found that at the pole #2 of the 10kV nitroum umbrella line, the front, left and right sides of the 10kV combined transformer burst, and the negative control terminal and the power meter in the meter box have been burned.

On May 11, 2019, Guangdong Shaoguan Ruyuan Power Supply Bureau experienced an outsourcing personal electric shock accident during the construction of a new platform area of the distribution network, killing 2 people.

The project volume of this project is a newly built 3-base tower and a platform transformer. It belongs to the bidding project of Guangdong Infrastructure and Distribution Network (Second Batch) Project (Second Section). Party A is Guangdong Shaoguan Ruyuan Power Supply Bureau.
Shaoguan Ruyuan Yao Autonomous County Yinquan Power Industry Co., Ltd. (a enterprise after the reform of employee shareholding of Shaoguan Power Supply Bureau) is the project construction unit. The two parties signed a contract on October 17, 2017.
2. Brief introduction:
: At 18:00 on May 10, Lin Moumou (labor dispatch), the squad leader of the 6th distribution team of Yinquan Electric Power Industry Co., Ltd. (hereinafter referred to as Yinquan Company), arbitrarily adjusted the line laying work of the #3-#1 pole line originally planned to be carried out on June 21 to May 11.

Li and Deng pulled the wire rotation head of the branch line #1 pole to below the low-voltage user line (about 50 meters), and then crossed the upper part of the low-voltage user line PVC pipe.
9:17, when preparing to release the second wire, the release plate failed. Liu left the user line low-voltage rod to check the release plate, and arranged for Li Moufang to pass the wire on the rod, Li Moumou was responsible for dragging the wire, and Deng Moumou was responsible for delivering the wire.
Li Moufang asked to follow the method of the first wire, but Li Moumou pulled the wire about 10 meters below the low-voltage line that was spanned by the distribution transformer, but before pulling it to rod #1, he turned his head and pulled it below the low-voltage line, and then crossed from above the low-voltage user line PVC pipe.
9:20, because the force on both ends of the wire was too high, Li Moufang took off his hand, and the wire ran off the wire, directly touched the insulated contact of the low-voltage user line and discharged, causing Li Moumou and Deng Moumou to be pulled.
