The fifth brothers are from Jiangxi. The third brother Wang Fusheng is 75 years old this year. He has settled in Hangzhou with his son for more than 10 years and rarely returns to his hometown. In August last year, Wang Fusheng developed stomach bloating and difficulty swallowing. "Even if he only had a few bites of supper, his stomach was bloating and uncomfortable. When he went to bed at night, he felt the food he had eaten was pressing against his throat." However, because he could tolerate it, he did not attract his attention. "When I came to the hospital for treatment, Wang Fusheng's condition was already advanced gastric cancer, and the cancer cells had lymph node metastasis. It was a pity. His condition was that he could detect cancer earlier and control the cancer cells in the bud."

Dr. Gong Weihua explained that the overall treatment effect of the tumor has a lot to do with the early and late stage, and early detection and early treatment is the key. At present, the 5-year survival rate after early-stage gastric cancer in can reach more than 90% after treatment. Once the progression period reaches the level, the 5-year survival rate will decline significantly. If the stage IV is reached, the survival rate will only be 10%.

"Early gastric cancer is equivalent to a newly sprouted seed, with a relatively limited growth range, and the effect of cutting weeds and roots is better; once the cancer cells spread and metastasize, they will spread to various organs like lush trees. Even if surgically removed, it will be difficult to remove all the tumor branches and leaves without recurring."

Although 80% of early gastric cancers have no symptoms, there are traces to Wang Fusheng, but he was careless and missed the best surgical opportunity. It turned out that during the interview, Dr. Gong Weihua learned that Wang Fusheng's eldest and second brothers both died of gastric cancer 3 or 4 years ago. Three of the brothers had cancer, and this is a typical familial gastric cancer.

"One of the high-risk factors for gastric cancer is heredity. The parents and siblings of first-degree relatives of gastric cancer patients have a risk of gastric cancer 2 to 3 times higher than the average population."

At the advice of Dr. Gong Weihua, Wang Fusheng's two younger brothers also had examinations in the local hospital. The youngest brother was also diagnosed with gastric cancer. Fortunately, it was in the early stage and has been discharged from the hospital.

The five brothers all love to eat high-salt foods such as pickled vegetables

This is also a high-risk factor that causes gastric cancer

Wang Fusheng brothers have lived in the border of Zhejiang and Jiangxi since childhood in Yushan . In the era of food scarcity, pickled vegetables are an indispensable and important dish. The economy has improved over the years, but high-salt and pickled vegetables such as sauerkraut, fermented tofu, and bacon have become part of their lives.

"High salt foods can destroy the gastric mucosa; nitrates in pickled products can be decomposed into nitrite and strong carcinogen nitrite . Nitrite and nitrosamine will induce cancer of the gastric mucosa, that is, mutations in cell genes, thereby increasing the chance of gastric cancer." Dr. Gong Weihua said.

Currently, the study also found that the intake of a large number of fresh vegetables and fruits reduces the risk of gastric cancer. "Vegetables and fruits contain a large amount of vitamin C. Vitamin C has strong reducing and antioxidant effects, and can inhibit the synthesis of nitriteamines."

In addition to dietary and living habits and genetic factors, precancerous lesions such as Helicobacter pylori and the stomach are also important factors in the high incidence of gastric cancer. Most currently believe that gastric cancer may be the result of a combined effect of long-term infection of Helicobacter pylori and other factors.

The infection rate of Helicobacter pylori in China is 40-60%, and some people will be infected again after treatment. The oral-to-oral transmission of bacteria in daily life is the main reason for the high infection rate. Therefore, to reduce the risk of infection, Dr. Gong Weihua suggested adding a pair of public chopsticks or separate meals during dinner.

gastric cancer is a "benevolent disease" in Japan

It is different from us talking about cancer. As Japan, which has a high incidence of gastric cancer, they unanimously believe that gastric cancer will not cause death and is a "benevolent disease". In fact, the mortality rate of gastric cancer in Japan is really low.

In 2017, Dr. Gong Weihua once went to Japan Cancer Research Hospital for further study for three months:

"I went there for three months, and there are three who have the deepest impression of Japanese medical treatment: First, they implemented the principle of bloodlessness in the operation, that is, they have never seen them transfusions during the operation, and they did not prepare blood routinely before the operation; Second, the operation is very rigorous, and the amount of bleeding during the operation is very small, and basically stop bleeding immediately when you see a drop of bleeding; Third, the early detection rate of gastric cancer is as high as more than 60%, and the highest center for early gastric cancer screening in China is less than 40%. "Why is the gap so big? Dr. Gong Weihua believes that the reason is gastroscopy, which is a routine physical examination item for people over 40 years old in Japan. In the cancerous mode of chronic gastritis-gastric mucosa atrophy-intestinal metaplasia-dysplasia-gastric cancer, gastroscopy, as the gold standard for diagnosis of gastric cancer, its advantage is that it directly sees the lesions of the gastric mucosa and biopsy , which can distinguish precancerous lesions and early cancers, and achieves early intervention and early treatment.

"The concept of gastroscopy in China has not been popularized yet. Most people also think of having gastroscopy after stomach discomfort and obvious symptoms. At this time, it is often advanced gastric cancer."

Dr. Gong Weihua said that if you want to stay away from gastric cancer, you must prevent it in advance. "For those over 40 years old, you can consider having gastroscopy every two years; if you have a high-risk group with a history of tumor genetics at home, especially those whose parents and brothers and sisters have gastric cancer, it is recommended to have gastroscopy once a year." Of course, you must also have a reasonable diet, eat less pickled and fried food, and do not eat moldy food; control drinking and smoking.
