Since the beginning of this year, I obviously felt that my body was "not very good", easily tired, hair loss, , I was afraid of cold in spring, sweated constantly in summer, and my throat was always very uncomfortable. As a voice worker, I always felt that I was overusing my throat and I didn’t care too much about it.

But worse, I had an abnormal and strange behavior - I, who has always been gentle, could hardly control myself from my husband when I was under great mental stress, and I could hardly control myself from my husband. Combined with the blood draw report of thyroid function , the doctor initially diagnosed me with "Hatmoto's thyroiditis (HT)" and suggested that I go to the endocrine clinic of Zhejiang University First Hospital for a look.

I have never heard of this disease! A few years ago, my grandma lost 40 kilograms in her hometown in Shanxi. She was diagnosed with "hyperthyroidism" and was hospitalized for treatment. I don't know if there are any family genetic factors, but my disease seems to be very different from "hyperthyroidism".

This disease is especially popular among middle-aged and young women. Hashimoto's thyroiditis was first discovered and compiled into a document by the Japanese Satoshi Hashimoto in 1912. Therefore, it is named Hashimoto's thyroiditis or Hashimoto's (Hashimoto) disease, also known as chronic lymphocytic thyroiditis.

But this "inflammatory" is not the other "inflammatory". It is different from inflammation caused by bacterial and viral infection. It is an organ-specific autoimmune disease. Due to the disorder of the thyroid autoimmune function, a large number of lymphocytes that should not have existed in the thyroid tissue ran into the thyroid gland, which in turn damaged the thyroid tissue.
For example, our thyroid gland is like a castle, and thyroid globulin and thyroid peroxidase are both the tissue structure of the castle. Due to the disorder of autoimmune function, the body produces " thyroid globulin antibody " and "thyroid peroxidase antibodies" targeting its own thyroid globulin and thyroid peroxidase. These antibodies are like enemy soldiers attacking the castle. They attack the thyroid gland frantically. Our thyroid gland can resist enemy soldiers in the short term, but over time, as the force expands, the thyroid gland will inevitably be damaged or even destroyed, which will lead to hypothyroidism or even loss.

Recently, the epidemiological survey results of thyroid disease and iodine nutrition completed by experts from the Endocrinology Branch of the Chinese Medical Association from 2015 to 2017 in 31 provinces and cities in mainland China showed that there are more than 200 million patients with thyroid dysfunction in my country, and the incidence rate is on the rise year by year. Hashimoto's thyroiditis accounts for about 7.3%-20.5% of all thyroid diseases. Statistical data also show that the prevalence of HT reported by Chinese scholars is 1.6%, and the incidence rate of women is 6.9/1000. If hidden cases are included, the prevalence rate of females is as high as 1/30 to 1/10. Women aged 30-50 are the highest incidence group, and the incidence rate of male and females is about 1:9.

Hashimoto thyroiditis will cause continuous damage to the thyroid gland and affect thyroid function, but this process is generally slow. According to the development of the disease, it is mainly divided into four stages:
Stage 1: The normal thyroid function period

Patients with normal thyroid function may have no symptoms, or have pharyngeal discomfort or mild difficulty swallowing; thyroid ultrasound will indicate uneven changes in the echo of the thyroid parenchymal echo; blood tests indicate increased anti-thyroid globulin antibody and/or anti-thyroid peroxidase antibodies.

Stage 2: Transient hyperthyroidism stage

Due to our immune dysfunction, the thyroid gland is attacked by autoantibody , causing the destruction of thyroid follicular cells. Excessive thyroxine is released into the blood circulation and causes transient hyperthyroidism. It is manifested as hyperemetic , easy fatigue, palpitations, insomnia, irritability, sweating, weight loss, etc. Some patients have anterior tibial mucous edema and protrusion . Generally, it only lasts for a few months, and many people may have passed this short stage without realizing it.
Stage 3: Hyperthyroidism hypothyroidism coexistence

There are symptoms of hyperthyroidism, and the blood test indicators may indicate that the thyroid hormone level is slightly higher, normal or decreased. The patient begins to show signs of hypothyroidism, such as anorexia, memory loss, drowsiness, dullness, dizziness, headache, constipation, depression, , weight gain, etc.
Stage 4: Permanent hypothyroidism stage

Due to the long-term destruction of thyroid tissue cells, the thyroid gland cannot maintain normal function. Patients may experience symptoms such as fatigue, drowsiness, edema, constipation, weight gain, and depression. After supplementing with thyroxine, some patients with hypothyroidism have normal test indicators, but they still feel discomfort in their necks. This may be caused by the long-term repeated inflammation destruction, resulting in the thyroid gland becoming hardened, shrinking or growing; some patients may experience transient elevated thyroid hormone levels and hyperthyroidism symptoms due to aggravation of inflammation. This stage is very likely to last for a lifetime.

Chief physician Dong Fengqin introduced that if patients with Hashimoto's thyroiditis have normal thyroid function and no discomfort, they only need to check the thyroid function and thyroid ultrasound every six months to a year; if hyperthyroidism or hypothyroidism occurs, they must take the medicine as precautions; if there is significant enlargement, pain, tracheal compression in the thyroid gland, and if the internal medicine treatment is ineffective, surgical resection may be needed, and long-term replacement treatment of thyroid hormone is still needed after the operation. At present, Hashimoto's thyroiditis does not have a special treatment, nor can it be cut off. What patients can do is to understand it as much as possible, "get along" with it peacefully, and minimize the harm of the disease. Women of childbearing age can still have a baby as long as their A-technology remains normal!

Hashimoto's thyroiditis has family aggregation, that is, it has a genetic tendency. Parents in the family have thyroid diseases, which not only refers to Hashimoto's thyroiditis, but also includes other thyroid diseases, such as hyperthyroidism, hypothyroidism, other types of thyroiditis, thyroid nodules, etc. As long as there are thyroid-related diseases, the chance of their offspring suffering from Hashimoto's thyroiditis will be greatly increased.

Excessive iodine intake
High iodine diet will increase the concentration of thyroid peroxidase antibody (TPOAb), induce and aggravate Hashimoto's thyroiditis, especially the increase in iodine intake can promote the development of clinical hypothyroidism in patients with implicit development. Therefore, patients should eat less foods rich in iodine. If you eat iodine salt, it is recommended not to eat or eat less seafood with high iodine such as kelp, seaweed, shrimp and shellfish, and try to avoid processed foods containing salt. If you choose iodine-free salt, the control of seafood can be appropriately relaxed.

Other factors
Such as selenium deficiency, excessive mental stress, excessive neck radiation, smoking, infection, obesity, etc. may also play a certain role. Therefore, patients need to quit smoking and lose weight, maintain their mood stability, and can appropriately eat selenium-rich foods (such as nuts, sesame, asparagus, fungi, eggs, freshwater fish). In addition, excessive tobacco, alcohol and coffee are not suitable.
