What is the thyroid nodule ablation without surgery
Executive Director of the Ultrasound Diagnosis Department of Zhoushan Branch of Shanghai Ruijin Hospital Liu Zhenhua introduced: Radiofrequency ablation of thyroid nodules is less damage than surgical treatment, but not all nodules are suitable for treatment through this method

What should I do if there is suspicious nodules in the thyroid gland? In addition to surgical treatment and drug treatment, there is now another treatment method. The reporter learned from the Zhoushan Branch of Shanghai Ruijin Hospital that radiofrequency ablation of thyroid nodules has become a mature treatment method. It was carried out in the Shanghai General Hospital in 2015. In 2020, Zhoushan Branch introduced this technology and has been treated for hundreds of patients.
Doctor suggested that he undergo radiofrequency ablation
At the end of 2020, during a physical examination organized by the unit, 26-year-old Xiao Jiang was found to have suspicious thyroid nodules on the right. So Xiao Jiang came to the Zhoushan Branch of Shanghai Ruijin Hospital for a follow-up examination. Ultrasound diagnosed the right thyroid nodule as TI-RADS category 4B, and it is recommended to have cytologic examination.
Liu Zhenhua, executive director of the Ultrasound Diagnosis Department of Zhoushan Branch of Shanghai Ruijin Hospital, performed a fine needle aspiration for Xiao Jiang under ultrasound guidance, and was diagnosed with papillary thyroid cancer after cytology examination.
Considering that Xiao Jiang is a young male and is prone to lymph node metastasis, he needs to be actively treated. The traditional treatment method is surgical removal of one side of the thyroid gland. Given that the malignant nodule is a small (4.2 mm × 4.0 mm) single nodule and no lymph node metastasis, it is fully in line with the evidence of radiofrequency ablation, Liu Zhenhua suggested that Xiao Jiang perform radiofrequency ablation, which can eliminate malignant nodules and preserve the thyroid gland intact.
After detailed understanding, Xiao Jiang decided to perform this minimally invasive surgery. After half a year of recovery after the operation, the nodule had completely disappeared by July 2021.
What is thyroid nodule ablation
Liu Zhenhua introduced that thyroid nodules are a common clinical disease, among which benign nodules account for the majority. It is complicated by various thyroid diseases, such as simple goiter, thyroiditis, thyroid tumors, etc. Its nodules are single or multiple, and there are clinically good and evil. The discovery rate of ultrasound examination is 20%-76%, of which about 5%-15% of the nodules are malignant.
The current treatment methods for thyroid nodules are: drug treatment, surgical resection, and interventional ablation. Drug treatment may have side effects and the efficacy is inaccurate; surgical resection can obtain the exact efficacy, but the damage is relatively large, and postoperative scars affect the appearance.
Interventional ablation is radiofrequency ablation under ultrasound guidance. It is a minimally invasive in situ tumor ablation technology that completely inactivates the tumor and its surrounding tissues through the principle of thermal damage to achieve the purpose of ablation.
Compared with surgical resection, interventional ablation can be said to be the smallest damage. The entire operation process is less than half an hour. There will be a small needle hole on the neck on the first day after the operation, and there will be almost no trace on the next day. In addition, ablation treatment does not require long-term medication.
It should be noted that within two years after ablation treatment, you need to go to the hospital for regular follow-up of ultrasound and thyroid function as directed by the doctor.
According to your own specific situation Choose the most suitable treatment method
At present, the indications of radiofrequency ablation treatment under ultrasound guidance: benign nodules—can effectively reduce the size of the nodules and improve compression symptoms; high-function nodules—reduce the volume of the nodules and control the thyroid hormone level; malignant nodules include tiny papillary cancer without local lymph node metastasis (
"No treatment method is better, only the more suitable solution is the best for different situations." Liu Zhenhua emphasized that not all thyroid nodules are suitable for ablation, such as malignant and multiple diseases greater than 10 mm. When patients choose treatment methods, they must choose the most suitable treatment method according to their own specific conditions.
As an island city, is the detection rate of thyroid nodules higher? Liu Zhenhua said that there is currently no research that Zhoushan people are more likely to suffer from thyroid diseases.I can only say that with the advancement of ultrasound technology, thyroid nodules are increasingly appearing in everyone's physical examination reports.
Liu Zhenhua reminds that once you find a thyroid nodule, you don’t need to be overly nervous, most nodules are benign. What citizens need to do is to have regular physical examinations, not stay up late or exercise more, develop good healthy habits, and not eat foods with excessive iodine content at one time, etc.
