In the early months of 2002, Senna continued to find himself competing for family performances at WWF where he and Shelton Benjamin and Tommy Dreamer and others fought. He continued to appear on the OVW show after the main draft in June 2002 until September 25 when he lost to Kenny Brolin in the Loser Leaves OVW game. He will later appear in a one-time appearance at Development Farm under the ring name of Mr. P in November for a six-player doubles match, where he teamed up with Hall of Famers Big Boss Man and Charlie Haas to win against Lance Cad, Trevor Murdoch and Sean O’Hare.

Thuganomics Dr. (2002–2004), on the June 24 Raw show, Vince McMahon ordered the entire lineup into the ring and declared that he needed them all to find some "ruthless aggression" from within himself, and that he was looking for one to stand out from the others. Cena made her debut on WWE on June 27 in an episode of SmackDown by answering Kurt Angle’s public challenge. After claiming that he has "ruthless aggressiveness", he lost the game with a suppression combination, but still performed well. After the game, Billy Kidman, Farouk, Rikish and the Mourner congratulated him. After nearly winning, Senna became a fan favorite and began arguing with Chris Jericho , beating him in revenge.Cena then had a fight with Los Guerreros (Chavo and Eddie Guerrero) and beat Chavo Guerrero on the September 7 episode of Velocity. In the September 12 episode of SmackDown, Cena and Edge teamed up against Los Guerreros, ending the feud in the process. He later went on to beat Albert and D-Von Dudley and others as he could only show up on Velocity for the rest of September.

Next week at SmackDown, Senna turned to attack Kidman and blamed him for their defeat, the first and only turn in his WWE career. In the Oct. 17 SmackDown episode, Senna beat Kidman in a singles match, but was eventually defeated by Kidman in the Oct. 24 SmackDown episode.
In the 2002 Halloween-themed SmackDown episode, Senna dressed up as Vanilla Ice and performed freestyle rap . The following week on SmackDown, Senna received a new character: a rapper who edited the promotional video while rhyming. Not long after, he earned the nickname of Dr. Thuganomics and expanded his gimmicks to include rap before the competition. He began wearing a hat and sweatshirt as part of his ring gear. In a documentary focusing on this period on the 2020 WWE Network, Senna revealed that at the time, the gimmick change to Dr. Thuganomics saved his career. Prior to that, WWE planned to launch Cena because the “ruthless aggression” gimmick wasn’t impressive enough. During a bus trip with other WWE superstars, Senna was involved in a freestyle rap conference with Rikishi and Rey Mysterio, impressing Stephanie McMahon, resulting in the adoption of this gimmick. As the character evolved, Senna began to adopt a variant of the 1980s WWF logo—remove the “F”—as his “signature symbol”, as well as the slogan “Word Life.” At Rebellion, he teamed up with Dawn Marie to lose to Kidman and Torrie Wilson in a mixed doubles match.
Cena eventually joined the executor Bull Buchanan, who was renamed B-2 (also written as B² and pronounced “B-Squared”). B-2 helped Cena beat Rikishi in 2003, the SmackDown episode on January 2 and the SmackDown episode on January 9 against Chavo Guerrero. Cena and B-2 both participated in the Royal Rumble competition, but neither of their attempts were successful. Cena and B-2 failed to win the WWE tag team championship from Los Guerreros, resulting in Cena replacing B-2 with Red Dogg until Red Dogg was sent to the Raw brand in February.
In the first half of 2003, Senna sought WWE championship and chased defending champions Brock Lesner after WrestleMania XIX, beating Eddie Guerrero, Mourner and Chris Benoit, who mocked Senna by wearing a "toothless aggression" shirt. He later won the No. 1 Contender Championship and won the right to compete against Lesnar in Backlash against the WWE Championship, but his attempt was unsuccessful. After Backlash, Cena started a feud with Rhyno and Benoi, causing Cena to defeat Spanky, Rhyno and Benoit's team on Judgment Day with Chuck Palumbo and Johnny Stamboli. Shortly thereafter, Cena started to laugh at The Undertaker, resulting in a singles loss in a game at Vengeance. But it won the rematch on the August 7 episode of SmackDown, this time with the help of A-Train. The dispute ended on the SmackDown show on August 21, when Cena and A-Train beat Undertaker and Orlando Jordan in a doubles match. Senna then competed in the U.S. Championship but lost to champion Eddie Guerrero in Latino street fighting in the episode of SmackDown on September 11.
After No Mercy lost to Kurt Angle, Cena joined Angle as a member of his team as a fan favorite, with Cena and Chris Benoit being survivors. Cena and Benoit join forces as interim partners, but on the Nov. 27 episode of SmackDown!, both entered a 20-person Royal Fight to win the WWE Championship. Cena and Benoit are the last two left, and they eliminated each other at the same time, resulting in the two being declared joint winners. However, Benoit beat Cena on SmackDown the following week! Win the WWE Championship match against Lesner that night.
